I'd suggest you shouldn't see professional and personal etiquette as one. (And discuss your wife with your patients at all, perhaps. ) Just as you've noticed, there are additional requirements/limitations at work that should be observed even if you would do differently otherwise.

Words don't hurt, inappropriate behaviour does. For example, a beggar addressed me at an airport. When declined, he made a verbal assault. It was hypothetical that someone would say something negative about my wife. It has never happened, but only a "what if" situation.

In any case, hitting another person over words is uncalled for. Period.
