Drive cycle clarifications

I have a few question to clarify how the drive cycle works.
Please don't answer if you're not 100% certain of the answer...

-When does a drive cycle starts ? when you start the engine or when you meet the speed / situational requirements ?

-Does the engine HAS to be cold to start a drive cycle even if I'm not doing the evap cycle ?

-If you exceed the threshold, (3K RPM, 60MPH) can you just keep going and try again or do you need to pull over, turn off the car and turn it back on to reset the cycle?

-If you have 3 out of 4 monitors ready, can you only do the part of the drive cycle related to the monitor that's not ready ?
