IRP disk bushing destroyed

i had mine for 4 months and when i was changing may transmission oil i saw my flex disk bushing blown..what are the odds of this happening? i couldnt get the better pictures for the other ones..

Revshift guibos are known for this, replace this with an oem piece and move on.
