A design flaw (fixing crankgear to shaft) which resulted in a bunch of aftermarket fixes for people trying to make more power (the common victims of failure). To my knowledge, none of the solutions are sold with a guarantee.

Gear slips off the post and causes timing chain to slip. Bad for interference engines.

Is this a common failure? My car is relatively stock, it only has downpipes and is on E85, no meth. Stock turbskies. Is it for higher mileage or pre-lci cars?

My buddy's CS had a failure before 10k mi (bm3), my CS was stock and tracked often with no failures. It could have happened on a stock car before but I think those cases are extremely rare. The only thing we know for sure 100% is the more torque you're pushing, the more likely it is to happen. On e85, you're at high risk in my opinion |

It's not a common failure. If your tuned then yes it will fail. Stock engines won't fail. If it does, it's like under 1% failure rate
